(If you are a Medicaid/SoonerCare subscriber and wish to switch to our facility for your PCP, you will need to call the number on the back of your card and tell them you want to change your provider to "Hooker Medical Clinic".)

If your insurance is not listed above, we can attempt to file, but it will be considered "out-of-network".
